How to Refer

Send referral letter, relevant records and specific questions to our secure email. Our team will confirm acceptance and schedule an assessment.

Required Information​

  • Letter of Instructions with referral questions

  • Claimant details (Full Name, Date of Birth, Home Address, Mobile Phone Number and Email Address)

  • Claim or case details

  • Contact person / Claims officer details

  • Relevant medical and service history

Appointment Bookings​

  • Once referral is received, our team will confirm doctors availability with the Referrer

  • Once agreed to, an appointment will be booked and confirmation email sent with the date, time, type of assessment, doctor details and the video-consultation link, to the Claimant and the Referrer

Turnaround & Communication

  • Prompt scheduling and confirmation

  • Reports delivered within agreed timeframes

  • Secure electronic delivery​
